Professionals in Home Care

Our senior management team possesses a collection of talent that is unmatched in the field of elder care, including geriatric care management and Alzheimer's care. With an average of more than 15 years of experience, these health care professionals bring to Practical Care Continuum a level of expertise that spans many elder care related professions. This includes social work, geriatric care management, long-term care, nursing home leadership, certified home care, and program design. Many services, such as social work and geriatric care management, have always been an intergral part of Practical Care's management philosophy.

Samantha S. Young
LNFA, Owner/Administrator

Samantha S. Young is an innovative entrepenuer with over 17 years of hands on experience, a degree in Health Professions from Texas State and is a licensed Nursing Facility Administrator. After ten years of working in a facility and running a successful non-profit program, Samantha decided that helping older adults remain independent in their own home and community was her personal mission in life and with that vision, opened her own home care business.

Samantha has operated, Practical Care Continuum, a successful non-medical home care agency in Austin, Texas since 2000. Her knowledge of the industry, business savvy, personal connections and willingness to share with others has proven to be a solid foundation for building her business.

Samantha is an active member of the American Society on Aging. As a company, Practical Care Continuum is a member of the Texas Association of Home Care, the Women's Chamber of Commerce and the Austin Chamber of Commerce.

As a member of the "Sandwich Generation" Samantha understands the challenges and rewards of caring for a parent and juggling personal and professional responsibilities.

Laura McManus, LBSW,
Administrator Round Rock office

Laura is a licensed social worker with over 17 years experience working in the medical/geriatric field including hospital, nursing home and home care settings. Prior to joining Practical Care in 2003, she directed the CARE Program at the Alzheimer's Association in Austin where she provided case management and coordination of client services.

Laura and her husband live in Round Rock and both are volunteers for the Round Rock Police Department. She served on the board of Faith in Action Caregivers in Round Rock from 2006-2007.  Laura is a 2004 graduate of Leadership Round Rock which involved increasing leadership skills and knowledge about Round Rock's and Williamson County's government, business, and non-profit services. She was the recipient of the 1999 Volunteer of the Year award from the Alzheimer's Association. She is also involved in local healthcare groups and has been a past officer with the Austin Area Medical Social Workers Committee, the Austin Area Home Health Council, and currently coordinates the Williamson County Health Resource meeting.

Laura is responsible for client assessments in Pflugerville, Round Rock and Georgetown. Laura also regularly provides educational in-services to professional and community groups.

Elke Gasch, LMSW,
Care Manager and Assistant Administrator Austin

Elke is a Licensed Master Social Worker who has a heartfelt commitment to helping older adults and their family members. Her love of helping seniors is inherited from her mother who sees many elderly clients in her beauty salon in Wisconsin. When she was young, Elke would spend Saturdays at the salon and would sit and talk with the “ladies” while they had their hair fixed.

Elke has over 10 years of experience working with seniors in a variety of settings. These include inpatient and outpatient geriatric mental health settings, a day program specifically for people with Alzheimer ’s disease and related dementia, a nursing home and as a geriatric care manager. She joined Practical Care in April 2007. She is currently the Speakers Chair for the Austin Area Medical Social Work Committee, a member of the Cognitive Support Coalition, and active in many other professional healthcare organizations. Her professional experiences have enriched her understanding of the complexities of aging and the importance of looking at each client individually to determine their unique needs and what will best meet their quality of life. Elke is responsible for client assessments in central and South Austin and Lakeway.  Elke is also available to provide educational in-services to professional and community groups.
