| Practical Care Continuum provides practical assistance to adults 55 and older in their homes, including Assisted Living Facilities, Nursing Homes, and Hospitals.
Services begin with an in-home assessment of both the older adult's needs and family issues. The assessment is an in-depth evaluation of the older adult's medical, emotional, and social needs. This process allows us to provide for each clients specific care requirements, which will enable them to remain independent and in their own homes.
Our philosophy is to provide quality care at reasonable rates while providing a living wage to our employees. This allows us to hire only the finest people who are caring and sensitive to the needs of older adults.
